Camp Connection Release of Liability Waiver & Required Information

Release of Liability and Assumption of Risk:​

I, the undersigned, acknowledge that participation in the activities at Walk with Autism Camp Connection Summer Camp ("the Camp") involves certain inherent risks, including but not limited to physical injury, illness, or even death. I understand that the Camp will make reasonable efforts to ensure a safe environment, but I acknowledge that such risks cannot be eliminated entirely.

I voluntarily assume all risks associated with my participation in the Camp and its activities, which may include but are not limited to sports, swimming, hiking, outdoor adventures, and other recreational activities. I agree that I am physically capable of participating in these activities.

Waiver and Release:

I, for myself, my heirs, executors, administrators, and assigns, hereby waive, release, and discharge Walk with Autism Adaptive Summer Camp, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, or causes of action that may arise from my participation in the Camp. This release includes, but is not limited to, claims for personal injury, property damage, or death caused by negligence or any other cause.

I further agree that I will indemnify and hold harmless Walk with Autism Camp Connection, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, or causes of action resulting from my actions or the actions of my child (if applicable) during the course of participation in Camp activities.

Medical Authorization:

In the event of an emergency, I authorize the Camp staff to seek medical treatment for me (or for my child, if a minor), including but not limited to first aid, emergency medical services, or hospitalization, if necessary. I agree to assume all medical costs and expenses associated with such treatment.

Authorization for Transportation

I authorize Walk with Autism Camp Connection to provide transportation for my child to and from camp and related activities during June 8 - June 12. I understand that transportation may be provided via SUV and all reasonable precautions will be taken to ensure the safety of my child.

I acknowledge that participation in camp transportation involves inherent risks, including but not limited to vehicle accidents, injury, or delays due to unforeseen circumstances. I release and hold harmless Camp Connection, its staff, drivers, and affiliates from any claims, damages, or liabilities arising from transportation, except in cases of gross negligence or misconduct.​​​​​​

I understand that if my child requires a car seat or booster seat under Utah law, I am responsible for providing one that meets all legal safety standards. I agree that Camp Connection staff may assist in securing my child in the seat but assume no liability for improper installation or use. If a required seat is not provided, I understand that my child may not be allowed to participate in camp transportation.

Photographic Release:

I consent to the use of any photographs or videos taken during the Camp activities for promotional or educational purposes without compensation.

Crawdad Canyon and The Farm at Zion:

My child can participate in the activities provided by Crawdad Canyon and The Farm at Zion. Waivers are located on the "Camp Schedule" page and MUST be completed.

Acknowledgment of Understanding:

By signing below, I acknowledge that I have read this Release of Liability and Assumption of Risk form. I fully understand its contents and the risks involved. I understand that I am voluntarily waiving certain legal rights, including the right to sue, and I agree to be bound by the terms of this agreement.
